• EFFECTIVE IMMEDIATELY: New HUD Form – 92006 - Must be used
JUST published the HUD Form 92006: Supplement To Application For Federally Assisted Housing
This form must be given to every applicant but its completion is optional. The form gives applicants the option to include the name, address, phone number, and other relevant information of a family member, friend, or person associated with a social, health, advocacy, or similar organization; Who is familiar with and may assist with the services and special care needed by the individual or family, and who can assist in resolving any tenancy issues arising while the resident lives on the property.
It has been a requirement to notify applicants that they have the option to provide this information for years. However, HUD has discovered that applicants are not consistently notified of the option to provide this information. Accordingly, HUD has determined that the best way to ensure that owner/agents comply with this requirement is to mandate compliance through utilization of a standard form to notify applicants of their option to provide the information.
Even though most owner/agents have always required an emergency contact form, some owner/agents have not; therefore; all owner/agents now have a requirement to provide HUD Form 92006 at application.
Specifically, HUD Form 92006 must be provided once, with each application for occupancy, in a HUD-assisted property. Owner/agents may not require the applicant to provide the information. Establishing procedures to demonstrate that the form was provided will help you avoid findings during your next MOR.
The supplemental information is to be maintained by the owner/agent as confidential information based on your privacy and security policy.
Download the form from the HUDClips web site and review the form instructions. Effective immediately, this form must be provided to all property applicants. Application packages should be updated appropriately. www.hud.gov/offices/adm/hudclips/forms/hud9.cfm
